To request Proof of Current or Former Registration and Licensure, access the portal here. Log in to the portal using your email and password. Go to the “My Forms” section on the home page of the portal and select the ‘Proof of Current or Former Registration and Licensure Form’. After you submit this form and payment, we will issue a letter confirming your registration and licensure status with NSCN.
Typically, this letter is requested for employment, education or immigration reasons. If this information is required by a regulatory body, NNAS, or CGFNS, please apply for a Verification of Registration Form.
Fees are in Canadian funds, includes the Harmonized Sales Tax (HST), and must accompany your request. We will complete this request within 15 business days once we receive both the request and payment.
