To request Verification of Registration and Licensure, access the portal here. Log in to the portal using your email and password. Go to the “My Forms” section on the home page of the portal and select the ‘Verification of Registration and Licensure Form’. Fill in the form to submit your request and pay the applicable fee.
After you submit this form and payment, NSCN staff will send a message via the portal and we will issue to the regulatory body of your choice a letter confirming that you currently or previously held registration and licensure with NSCN. This letter is sent to other regulatory bodies, NNAS, and CGFNS only.
Typically, this letter is used when you want to register or license in another province or state.
Fees are in Canadian funds, includes the Harmonized Sales Tax (HST), and must accompany your form. We will complete this request within 15 business days once we receive both the form and payment.
If you want to have a verification letter sent to multiple regulatory bodies, please add the name of each regulatory body to the “Regulatory Bodies” field on the form.
