Health Assessment: Self-Assessment Guidelines

A health assessment is the process of collecting, verifying, and organizing information about a client within a practice context. LPNs use client data and collaborate with other health care professionals to contribute to the development of the client’s plan of care. This data is also used to make decisions in the selection, continuation, change and implementation of nursing interventions or plan of care, and recognition of changes in client status. We developed this practice guideline to support LPNs to perform a self-assessment of their individual competence related to health assessment.
